Overview
Cluster headaches are among the most painful types of headache. Attacks often happen at the same time each day and can wake people from sleep.
Symptoms
- Severe, stabbing pain around or behind one eye
- Attacks lasting 15 minutes to 3 hours
- Red, watery eye and drooping eyelid on the same side
- Blocked or runny nostril
- Restlessness and agitation during attacks
Causes and risk factors
The exact cause is not known but involves the hypothalamus in the brain. It is more common in men and smokers. Alcohol can trigger attacks during a cluster period.
Diagnosis
Diagnosis is based on the pattern of symptoms. An MRI may be done to rule out other causes.
Treatment
Attacks are treated with high-flow oxygen or triptan injections or nasal sprays. Preventive medicines are taken during cluster periods.

See a doctor if you have severe headaches around one eye. Seek emergency care for a sudden, severe headache that is unlike any before.
