Overview

Children with selective mutism speak normally at home but not at school or with unfamiliar people. Social anxiety causes intense fear of being judged. Both respond well to early treatment.

Signs

  • Consistently not speaking in certain settings
  • Fear of meeting new people or performing in front of others
  • Clinging to parents or avoiding social events
  • Physical symptoms such as stomach aches before school

Causes and risk factors

Risk factors include a shy temperament, family history of anxiety, speech or language difficulties and stressful experiences.

Diagnosis

A child psychiatrist or psychologist makes the diagnosis after assessment. Hearing and speech should be checked.

Treatment

Treatment includes behavioural therapy with gradual exposure, family and school involvement and, in some cases, medicines.
